Each child must be between the ages
of 3 and 13 AND enrolled between grades preschool and fifth grade beginning
September 2013

Limit to one pair of shoes and one
pack of sock per child
o
MUST VERFIY GUARDIANSHIP WITH STATE
ID & CHILD’S BIRTH CERTIFICATE at your appointed date/time of pickup
(originals shown, no copies)
o
Items cannot be held for a later
time; items not retrieved at your appointed
date/time are donated to others
o
No information will be collected on
site; info is used for verification purposes only

Supplies are limited and will vary,
so compliance to the following is required to receive the donated items:
